Registration For GP'S

Please see below our application pack for you to download and complete in order for you to be able to work with us. Once you have completed this please return to us along with ALLsupporting documents listed below.

• Completed Application Pack (including App Form, Starter Form, Terms of Engagement, Fitness to Practice)
• 2 Passport sized photos (for ID Badge)
• Copy of CV
• Passport/ Birth Certificate
• EU Passport, work permits or Visa disclosing right of abode (If applicable)
• Proof of National Insurance Number (e.g. NI Card, P60 or Old Payslip)
• Proof of Address X2 must be less than 3 months old (e.g. Utility bill or Bank Statement)
• GMC Certificate
• Professional Indemnity Insurance (e.g. MDU, MDDUS or MPS)
• Copy of JCPTGP, PMETB or CCT
• Copy of Performers List
• Copy of Hepatitis B Serology Report or Booster (must be 5 years in date)
• Fully Enhanced CRB with POVA and POCA checks within the last 12 months. It is our standard practice to initiate a CRB from Impress Recruitment Services Ltd
